Grampian Heart & Health provides, coordinates, promotes, encourages and manages phase IV Cardiac Rehabilitation (CR) and chronic health conditions (CH) exercise classes throughout Grampian so that long term benefits may be widely available to members in the community. This is done by establishing partnerships and referral networks with local GP's, health centres, hospitals, physiotherapy services, diabetic clinics, phase III and other healthcare providers to support the referral of individuals into appropriate programme in the community. Providing a means for gathering appropriate CR and CH data for potential study and research to assist in the identification of optimum exercise. Gathering and providing funds for ongoing professional development, accreditation and guidance for all staff and volunteers involved in CR and CH, so small groups may benefit from association with larger groups. Providing defibrillators for first aid to CR and CH participating members.
