To promote social inclusion by (1) preventing people from becoming socially excluded, (2) relieving the needs of socially excluded people and assisting them to integrate into society and (3) preserving protecting the health of socially excluded patients of Cleobury Mortimer Medical Practice (through providing resources not normally provided by the statutory authorities).
