To provide the care and support which enables patients served by 8 surgeries in Derbyshire Dales and 14 surgeries in Sheffield 5, who are suffering from a life threatening illness to stay at home. Such services to be provided without charge to patients or their families,and shall be provided in close consulation with other care professionals so long as they do not replace any statutory obligation
